


Costa Salud Community Health Centers, Inc. (CSCHC) is a private non-profit corporation that offers primary and preventive health services in Rincon, Aguada, Moca, and surrounding municipalities. Since 1975, we have served primarily low-income people with limited medical care access. Our commitment is to offer our patients excellent health services, being responsible for managing their health care in an integrated way and sensitive to their needs.
At Costa Salud, we are committed to your health and disease prevention. For this reason, we strive to detect the risk of suffering from conditions caused by genetic inheritance, uncontrolled habits, or poor-quality lifestyles at work or home. We have an engaged medical faculty staff and special programs for the well-being of our patients.

Throughout these five decades, we have worked with passion to provide an excellent service focused on prevention, comprehensive care, and the humane treatment that characterizes us.

Our Board of Directors is mainly composed of users of CSCHC services, who responsibly delegate the implementation of the corporate policy to the Executive Director and his/her teamwork, considering the needs of the population served.
FEDERAL TORT CLAIMS ACT
Costa Salud Community Health Centers is a deemed covered entity under the Federal Tort Claims Act (FTCA) by the United States Department of Health and Human Services (HHS)/ Health Resources and Services Administration (HRSA), which covers the organization and its employees regarding to health or health-related claims, including medical malpractice claims within its scope of services.

Patient Centered Medical Home

‘’PCMH is a patient-centered healthcare model. The care provided by this model strengthens the doctor-patient relationship, replacing periodic visits with coordinated, long-term care. Each patient will have a relationship with their primary physician and healthcare team that is personalized, coordinated, effective and efficient. PCMH transforms the way we view clinical care, focusing on the primary care team, in addition to giving the patient an essential role in their health care. CSCHC's team of health professionals uses the American Medical Association (AMA) health care guidelines as the basis for their health care. In addition, it is governed by the highest quality standards and regulations of the Federal and State Government.”
